How We Work With Referring Physicians
We offer three flexible referral pathways to support your patient and align with your clinical goals. Providers may submit a direct treatment order, collaborate with our team on a custom treatment plan, or refer patients directly to a Terrain Health provider for ongoing care.

Case Collaboration with Custom Treatment Plan
Direct Referral to a Terrain Health provide
Direct Treatment Order
(Terrain provides only the treatments you specify.)
This option is ideal when you already have a diagnosis and treatment plan and simply need a trusted partner to administer IV therapies or other treatments.
How it works:
You complete the Terrain Health Order Sheet (Need sheet) with the specific treatment protocol.
You send us the completed order, relevant labs, and patient demographics.
Our team contacts the patient to schedule treatment.
Terrain Health administers only the treatment you have ordered.
There is no Terrain Health provider evaluation, consultation, or follow up visit.
This pathway keeps you fully in charge of medical management while using Terrain Health as your treatment site.
Case Collaboration with
Custom Treatment Plan
(Shared decision making while you remain the primary physician.)
This pathway is best for complex cases where you want collaborative support from Terrain Health while retaining full responsibility for patient management.
How it works:
You send us the patient’s medical records and recent lab work.
We schedule a peer to peer consultation between you and a Terrain Health physician.
During the consult, our physician reviews the case, may recommend additional testing, and creates a custom treatment plan.
You discuss the plan with the patient and determine whether to move forward.
If the patient agrees, Terrain Health provides treatment administration only.
All follow up care, medication adjustments, and routine management remain with you. Patients return to you after each treatment.
Peer-to-peer Consultation Fee:
Dr. Rose: 1,200
Dr. Fraser or Caitlin Policastro, APRN: 600
Fee may be billed to the patient or to your practice. Please indicate your preference when referring.
Direct Referral
(Full transfer of care to Terrain Health.)
Use this pathway when you prefer Terrain Health to assume full management of the patient’s condition. This is appropriate for complex chronic illness, Lyme disease, mold illness, autoimmune disease, Long COVID, and similar conditions.
How it works:
You send a standard referral and relevant records.
The patient is scheduled for our two part new patient evaluation, which includes an in depth consultation followed by a lab review and treatment planning visit.
The patient establishes care with a Terrain provider who manages all ongoing follow up, medication updates, and coordination of care.
We can share updates on the patient’s progress if you would like continued communication.
